The population served by Healthy Start is primarily composed of single, white women of low socioeconomic status. Many of these women have less than a high school education and are unemployed. Moreover, they mostly live alone with little, if any, social support. Age varies among participants.DESCRIPTION OF HEALTHY START SERVICESClients orders are referred to the smoking cessation program in one of two ways. When a woman in Pinellas County goes for a routine prenatal visit, she is presented with a Healthy Start screening form that she is asked to complete. If she identifies herself as a current smoker on the form, she is then offered the smoking cessation services. The other method a client is recommended to the program is by way of referral from a case manager. Women receiving other services offered by Healthy Start, who have already established themselves with a case manager, may necessitate smoking intervention and are, thus, also advised of the program. Once the referral has been made, the client is contacted by phone within five days by the smoking cessation health educator, who schedules a home visit to establish face-to-face contact. The home visit is to take place within ten days from the date of the phone conversation.

In the present prospective, randomized, double-blind study with 144 smokers, 23.7% of the patients receiving placebo stopped smoking, compared to cigar 55.9% of the patients receiving nortriptyline (p < 0.001). Individuals were considered to have stopped smoking if they claimed total abstinence from cigarette smoking for at least 1 week. Our results were comparable to those of another randomized study by Hurt et al, (16) which included 615 patients who had been treated with bupropion. In the sixth week of the study, the abstinence rate was 19% for the group receiving placebo and 44.2% for the group receiving bupropion (p < 0.001).Prochazka et al (18) in a randomized, double blind, placebo-controlled study with 214 patients, evaluated the effects of nortriptyline on the rate of ceasing to smoke. The success rate for the group that received the active drug was 50%, compared to 32% for the group receiving placebo. Only 12% of the patients had a history of depression, compared to orders 48% in the population of our study. Despite the difference between the two studies in the methodologies used to obtain this information (in our study, the report on psychiatric treatment for the diagnosis of depression was considered as previous history of depression, while in the study by Prochazka et al, (18) a structured questionnaire was applied [ie, the diagnostic interview schedule]), the difference between the two populations could explain the different in the rate of success in ceasing to smoke.
